CRUD Sql Server Custom database

I have created a table in sql server and interfaced with joget using custom datasource am able to show the data in the data list. I just dont know how to perform the Create update and delete on the same table . Here are some snaps

How can i create record in this table from joget form i have used the jdbc load and store binder with following queries but no luck... Any help will be appreciated.
