Issues in API

I have created one process for calling API I have set a form and configured it's field to show data 

but when i am calling that process I am getting same output which I have configured in JSON tool

Turn on debug mode in Configure JSON Tool properties to see what is the JSON Object Name

After making the changes as per your suggetion I am getting this error in console

curious...what version are you using? I did it on 7.0.30 enterprise and had no errors. Also, we'll need to see the log error from the top/beginning. Usually it begins with a timestamp

You know what? try my sample app and see if you can use it as a reference. APP_json-test.jwa. Launch the app > go to manage form > New > Save (don't need to put any values)
