PDF file viewer not working

- I followed the guide from this link : PDF File Viewer inside Form
But after uploading my pdf file , it does not appear as an attached file meaning(the HTML code requires it to appear as a "a" tag with href attribute but it appears as a span.
The function will not trigger even after doing this:
forms;userviews;user;datalists
