conditional statement

i want to ask if it is possible for me to have a if-else or other conditional statement in this window

attached is the picture below.

 

when the user checks the checkbox value "GDS"

the Username and Password in the "For Emails Only" will be visible or will be enabled. 

what can i use to make this happen?
