Process Trail not working

Hi,
Currently I have an app which uses process trail in the workflow, im trying to store the approval status done by the user/approver for each form but its not working as it is not getting inserted into the DB and nothing is showing up in the process trail element.
Logs file says nothing about inserting into the DB. I have no form that this table call lg_pt_legal_request but i created one in the DB.
