How to customise bootstrap formatter to view it on the top of the form?

This is my form. I want the Draft, Submitted, Scheduled, etc. to be viewed in the blue top bar. Please suggest how to modify the source code to view the bootstrap formatter in the following manner.

Hi, I am not familiar with the bootstrap formatter, but you should be able to modify the UI using custom CSS or JavaScript. In your Userview theme, add the custom CSS or JavaScript under the Advanced properties.
