Thank you Matthew for you kind support. I've downloaded both apps that you suggest above but none of them meet my need. The close one is the "User Profile" menu from EMPLOYEE SERVICE PORTAL, but i can't add another fields to it. Is there an example how to custom create "User Profile" like in the EMPLOYEE SERVICE PORTAL ?

Saman Sjukur You can copy the 'user profile' form from Employee services portal to your app if you like. Do also download and try this new app "Account Creation App" in https://marketplace.joget.org/jw/web/userview/mp/mpp/_/vad?id=APP_AccCreation-1. It is a complete workflow app (with approver form) for registering new user accounts in Joget Workflow. Don't be afraid to download and try out/learn all the apps from Joget Marketplace. There is no limit, you can download as many times as you like. You might find a ready app that suites your requirement without writing any new apps :-).
