Simple Purchasing error

I installed the "simple purchasing" app, when testing it i try to go through step1:
STEP 1 - Setup your Company information
all things OK, i can enter all required parameters, but when i try save, i get the error message "Validation Error"
i put in every field "Custom Error Message to show when Validation Fail" the name of the field, but it seems to be general error
