I want to create a form where users may select whether they want to fill out the Application Form OR the Infrastructure Form under CHANGE REQUEST FORM. By the way, the contents of the Application Form and the Infrastructure Form are the same.

Then, how can we make the Application Form or the Infrastructure Form that the user selects show up in the List?

Hi, if the forms are the same you can use Subform within a Multi Paged Form, perhaps with just a hidden field in each form to capture the type. For the list, you can just use the Form Data List Data Store to retrieve the records.
