Unable to receive Email when multiple levels exist

This is an Incident Report request. We created the forms and processes for the same. Requester submits the Incident Report form and reaches to the next level (Group Leader). Once Group Leader approves the request, it reaches to the another Level  (QA Review) and so on.

 

Now the issue is that we are not receiving email once Group Leader approves the request. Kindly advice.

Your Group Leader lane appears to have two Approvers activity boxes.

Maybe you could organize your workflow as follows, with clear approved and rejected email tools:
