Record not found when we trigger latest process flow

Hii there

We have an app with 4 version when we try to trigger process flow on latest version it shows record not found. but when i just changed the version in url to previous it works but as i mentioned version 3 has not the latest changes in flow. Please find attached screenshot for your reference.

Looks like either the form don't exist or the process not mapped to the right form. Do check.
