How to create custom activity instance?

let say, in 'Terima Arahan' user will accept task that already addressed to him/her. After user accept their task, 'Maklumat Pemantauan' Activity instance will be created base on how many location that in the task. For example, if there 3 locations in the task, so in the user inbox will have 3 'Maklumat Pemantauan' activities instances with different location.
